Your copayment (amount of session fee which you are responsible for) varies by plan. It is your responsibility to understand your copayment and insurance benefits. You may pay copayments by credit card, personal check or bank transfers through Zelle®.
Please note that some insurance plans (usually PPO plans) have a deductible that needs to be met before the insurance plan will cover the cost of sessions. This means that you are responsible for paying out of pocket until this limit is met. Example: if your deductible is $1500, you will have to pay that much in session fees or other medical expenses before your insurance takes over payment for sessions. Depending on your plan, you may or may not have an additional copayment per session after the deductible is met. It is your responsibility to understand your deductible, if applicable. These types of deductibles are not standard and only apply to select insurance plans.
I do not bill out-of-network services for other insurance plans, however I am happy to provide receipts for patients who want to submit them to their insurance for partial reimbursement (when allowable by plan).
I offer private pay options for patients who do not want to use their insurance or do not have an eligible insurance plan. My rates are:
I also offer a sliding scale for patients who wish to pay privately and cannot afford the full session fee. In this case, we will negotiate what you can afford based on your financial status.
In accordance with Section 2799B-6 of the federal No Surprises Act, health care providers and facilities must provide a Good Faith Estimate of treatment costs for underinsured or private pay patients. This notice does not apply to patients who use in-network insurance policies to cover the cost of sessions. The Good Faith Estimate applies only to patients who do not have health insurance, who do not have a qualifying insurance plan that I am in-network with, or do not plan to file claims with their insurer. In such cases, I will provide an estimated breakdown of costs for treatment for the current calendar year. This estimate may be revised if my rates change or if the anticipated length of treatment changes. Please note that if your costs for treatment are significantly higher than expected, you have the right to dispute the bill with the US Department of Health and Human Services.
